Mexico to charge police officers in attack against CIA agents

After an ambush that injured US CIA operatives, the Mexican government has indicated they are close to charging police officers, who they say may be corrupt.?

Security officials identified the men as CIA agents, and Mexican investigators have said the attack may have been carried out by corrupt police working with drug gangs.

The incident, which police first blamed on a case of mistaken identity, was the worst attack against US officials in?Mexico?since drug gang hit men killed a US immigration agent and wounded his colleague in a highway attack in early 2011.

Mexico's attorney general said police were likely to be prosecuted for the ambush of the two agents as they traveled in a vehicle with diplomatic plates on a road south of?Mexico City.

"At the moment everything points to the fact that there will be people to charge," said attorney general?Maries Morales. "We're now carefully analyzing the conduct of each officer to ascertain what exactly they can be charged with."

The CIA has declined to comment on the incident. Mexican authorities have detained 14 people in connection with the case.

Analysis: What's eating Australia? Foreign buyers at the farm gate

SYDNEY/CANBERRA (Reuters) - Australia risks losing an opportunity to become a farmyard for Asia, as growing unease over foreigners buying rural land threatens to provoke protectionist policies that may deter much needed investment in agriculture.

With its vast landmass, abundant natural resources and stable government, Australia has relied on foreign farm investments for more than 100 years, with interest set to grow as the world looks to dramatically boost food production to feed Asia's booming middle class over the next 40 years.

"I think we are going to see a continued interest. The soft commodities boom around the world and the demand for food will continue to drive that interest," said Jock Laurie, head of Australia's top farming lobby, the National Farmers' Federation.

The latest wave of interest in Australian agriculture has seen a number of high-profile deals involving Chinese investors, including the purchase of the country's biggest cotton farm, as well as foreign takeovers in its deregulated wheat industry. U.S. firm Archer Daniels Midland is bidding $2.8 billion for Australia's dominant grain handling company, GrainCorp.

But the growing interest has ignited a political debate and raised the risk of tighter foreign investment rules in a country generally seen as more open to investment than farming rivals Canada and New Zealand.

"We know foreign investment is important to Australia. But we need to make sure we're not selling the cow along with the milk," said independent Senator Nick Xenophon.

The issue is a sensitive one in mostly conservative rural communities, particularly for a government struggling in opinion polls and facing elections in late 2013.

A poll by the Lowy Institute think tank this year found four out of five Australians opposed the government allowing foreign companies to buy Australian farmland. Some 63 percent were "strongly against" such sales.

NATIONAL INTEREST

Most sales of Australian farms to foreigners go unregulated and unrecorded. Only the largest farm deals - purchases of at least 15 percent of properties worth more than A$244 million ($252 million), or A$1 billion for U.S. investors - are examined by the Foreign Investment Review Board (FIRB).

FIRB's "national interest" test has also been criticized as too vague and lacking in transparency. The rules mean foreign investment is assumed to be in the national interest unless the FIRB finds otherwise.

Foreign investment rejections are rare. In the 2010-11 financial year, FIRB considered 10,293 investment deals and rejected only 43, with 42 of those suburban real estate purchases. It approved 5,687 investments with conditions.

In Canada, foreign investors need to prove there is a "net benefit" to Canada for any investment over $332 million, and the industry minister has broad leeway to interpret the rules.

Ottawa has scuppered at least two high profile resource deals - BHP Billiton's $40 billion bid for Potash Corp and most recently, a $5.2 billion bid for Progress Energy by Malaysia's Petronas.

New Zealand reviews all foreign acquisitions of farmland above 5 hectares, meaning deals worth hundreds of thousands of dollars rather than tens of millions can be examined.

A lot is at stake for Australia.

The U.N. Food and Agriculture Organisation has said global food production needs to rise 70 percent by 2050. Australia, which exports around A$30 billion worth of food yearly, wants to cash in on that growth, particularly from China and India.

Yet a report compiled for ANZ Banking Group has found Australia needs an extra A$1 trillion in capital over the next four decades to improve agricultural supply chains and production and fund the transfer of farms to a new generation.

Though a modern farm economy, Australia struggles with under-performing properties, inefficient supply structures and high levels of farm debt and needs to invest in new technology, markets and products, ANZ said in its "Greener Pastures" report.

Australia is fighting not only its developed-world peers, but faces fierce competition from emerging market agricultural powerhouses Brazil, Malaysia and Indonesia, the report notes.

PRESSURE FOR CHANGE

The conservative opposition, which is on track to win power at the next elections, wants the farm threshold for foreign investment scrutiny lowered to A$15 million, and wants to make sure the FIRB has at least one member with farm expertise.

A Senate inquiry, dominated by opposition lawmakers and due to report in November, is likely to call for lower thresholds so more farm deals face FIRB scrutiny.

The government, reliant on Greens and independents to hold power, has so far been reluctant to make significant changes, conscious of the need to attract foreign investors to help build its farming sector.

Instead, Prime Minister Julia Gillard has agreed to set up a register of foreign ownership of farm lands in a move designed to appease public unease without unsettling investment flows.

But the political noise surrounding foreign farm investments continues, with the leader of the rural-based National Party, Warren Truss, criticizing the ADM bid for GrainCorp.

"We are rapidly descending into a state where farmers will toil in their paddocks while post-farm gate profits from Australia's A$9 billion a year grain crops will be counted in multi-national boardrooms," said Truss, who would become deputy prime minister under a conservative government.

Ian Smith, of corporate advisory firm Bespoke Approach which has helped Chinese investors tip-toe through the FIRB processes, said he did not believe the new register signaled a tightening of foreign investment rules.

"Given the recent political attention it is not a surprise that steps have been taken. It is not so much a tightening of the regime, but more a sign by government that it is responsive to public concerns without undermining potential investment," Smith said.

Foreign investors needed to be aware of the domestic debate, and make sure they highlight benefits to local communities and jobs from any deals, he said.

"The noise of the outspoken and at times xenophobic politicians is very damaging for Australia's reputation as place to invest," Smith added.

Emanuel Steward Dead: Famed Detroit Boxing Trainer Dies At 68

DETROIT ? Emanuel Steward, earnest yet easygoing, proved rough and tough wasn't the only way to win in boxing.

With a twinkle in his eyes, a smile on his face and a soothing voice, Steward developed unique bonds in and out of the ring with a long line of champions that included Thomas Hearns, Lennox Lewis, Oscar De La Hoya and Wladimir Klitschko.

Steward, owner of the Kronk Gym in Detroit and an International Boxing Hall of Fame trainer, died Thursday. He was 68. His executive assistant, Victoria Kirton, said Steward died Thursday at a Chicago hospital. She didn't disclose the cause of death.

"It is not often that a person in any line of work gets a chance to work with a legend, well I was privileged enough to work with one for almost a decade," Klitschko said Thursday. "I will miss our time together. The long talks about boxing, the world, and life itself. Most of all I will miss our friendship."

Steward, whose father was a coal miner and mother was a seamstress, was born in West Virginia. He got boxing gloves as a Christmas present at the age of 8, the start of what would become a long career in the sweet science.

He moved to the Motor City just before becoming a teenager and trained as an amateur boxer at Brewster Recreation Center, which once was the home gym of Joe Louis.

Steward, at the age of 18, won the national Golden Gloves tournament as a bantamweight. Instead of trying to make it as a professional boxer, he went to work for the Detroit Edison Co. and in 1971 accepted a part-time position as head coach ? for $35 per week ? of the boxing program at the Kronk Recreation Center.

A dynasty was born.

The Kronk's first professional champion was Hilmer Kenty, a lightweight from Columbus, Ohio, who started training there in 1978 and won the WBA title two years later.

But It was Hearns who really put Kronk ? and the trainer known as Manny ? on the map. The boxer known as Hitman was the first man to win titles in four divisions ? he won five overall ? and topped his 155-8 amateur record by going 61-5-1 with 48 knockouts as a pro.

Even though Steward had a lot of success with Hearns, some of his setbacks from his corner were among the most memorable in the sport. Hearns was knocked out in the 14th round by Sugar Ray Leonard in 1981 ? Steward said that was the most painful experience of his life ? and Hearns was on the short end of a three-round fight with Marvin Hagler in 1985 that is considered one of the best bouts in boxing history.

"He brought the very, very best out of me," Hearns once said of Steward.

Hearns wasn't alone.

Steward trained, helped train or managed some of the greatest fighters ? and some kids who just needed to get off the streets ? of the past 40 years out of Kronk and in other facilities across the globe, putting fighters from many countries in red and gold trunks.

"Lennox used to say when fighting as an amateur that everyone was afraid of the Kronk guys," Steward once said. "He saw the respect when they saw the colors."

The gym for years was seen as a way to keep kids out of trouble in southwestern Detroit.

"A lot of these kids would be in the streets," Steward once said. "They live for this."

And, Steward lived for it, too.

He loved boxing ? and boxers ? but like the Motor City, the gym he adored fell on hard times.

The city closed the original Kronk Recreation Center ? a hot, sweaty basement gym ? after vandals stole its copper piping in 2006. It was allowed to remain open, but it put Steward in a difficult financial situation and he later rented space at a gym in Dearborn so his young fighters could train.

"With the loss of Emanuel Steward, we have lost a true Detroit icon," Detroit Mayor Dave Bing said. "Emanuel Steward embodied our city's toughness, our competitive spirit, and our determination to always answer the bell.

"We are grateful for Emanuel Steward's many contributions to our city and his impact on generations of young people."

In the early years at Kronk, most of his fighters were black. In recent years, his melting pot of boxers included a Ukrainian heavyweight, an Irish middleweight and scores of young men from Eastern Europe, Latin America and the Middle East.

Steward was inducted into the International Boxing Hall of Fame in 1966. He worked closely with Lewis during his title run, and with current heavyweight champion Wladimir Klitschko as recently as July when the boxer led 22,000 fans in singing "Happy Birthday," to the beloved trainer.

Klitschko had to trained recently without Steward for his fight against Mariusz Wach next month in Germany

"His spirit is always here," Klitschko said. "I can hear his voice in sparring while doing things, whispering in my ear. As the famous saying goes, `The show must go on,' and that's exactly the case."

Steward also trained actor Wesley Snipes for his role as a boxer in "Undisputed" and appeared briefly with Lewis in "Ocean's Eleven

Steward worked since 2001 as a boxing analyst for HBO, sharing his rich knowledge of the sport and an adorable personality with a younger generation of fans.

"Manny was a respected colleague who taught us so much not only about the sweet science but also about friendship and loyalty," HBO Sports President Ken Hershman said. "His energy, enthusiasm and bright smile were a constant presence. Ten bells do not seem enough to mourn his passing."

HBO Sports commentator Jim Lampley ? a friend of Steward's since the early 1980s and broadcast partner for more than a decade ? said Steward's wisdom and common touch made him special.

"He was the most loyal, generous, positive spirited, broadly accepting man I've ever known," Lampley said. "He had a positive impact on everyone he encountered."

Satellite images tell tales of changing biodiversity

Analysis of texture differences in satellite images may be an effective way to monitor changes in vegetation, soil and water patterns over time, with potential implications for measuring biodiversity as well, according to new research published Oct. 24 by Matteo Convertino from the University of Florida and colleagues in the open access journal PLOS ONE.

The authors designed statistical models to estimate two aspects of biodiversity in satellite images: the number of species in a given region, or 'species richness', and the rate at which species entered or were removed from the ecosystem, a parameter termed 'species turnover'.

They tested their models on data gathered over 28 years in a water conservation area in the Florida Everglades and compared their results to previous reports from the region. They found that their models were nearly 100% accurate when predicting species turnover; conventional methods only have 85% accuracy.

According to the authors, their automated method using satellite images could help improve the efficiency and decrease the cost of campaigns that monitor biodiversity and guide policy and conservation decisions. Convertino says, "Texture-based statistical image analysis is a promising method for quantifying seasonal differences and, consequently, the degree to which vegetation, soil, and water patterns vary as a function of natural and anthropic stressors. The application of the presented model to other fields and scales of analysis of ecosystems is a promising research direction.''

(AP) ? What's real estate mogul Donald Trump got against President Barack Obama? The president says it's an old grudge that goes back ? way, way back ? to their childhood days in Kenya.

"We had constant run-ins on the soccer fields," Obama told "Tonight Show" host Jay Leno during a tongue-in-cheek knock on Trump Wednesday night. "He wasn't very good and resented it. When we finally moved to America, I thought it would be over."

Trump became a leading voice in the "birther" movement, the discredited but very much alive conspiracy theory that Obama was actually born outside of the U.S. and isn't constitutionally eligible to be president. Earlier Wednesday, Trump offered to donate $5 million to the president's charity of choice if he would release his college and passport records.

Wisecracks aside, Obama told Leno that he has never actually met Trump.

When Leno asked Obama if he was glad the presidential debates are over, the president responded, "You know, I was sort of getting the hang of it." Obama, whose performance in the first debate was widely panned, said he didn't do an effective job of energetically outlining the contrast of visions between him and Republican Mitt Romney.

Part of the problem with a debate, Obama said, is that it's not a natural way of communicating ? having an argument with someone as you sit next to him.

"Well," Leno said, "you're married."

Obama shot back: "But the difference is, with Michelle, I just concede every point."

Asked which team he was backing in the World Series, the Detroit Tigers or the San Francisco Giants, Obama managed to get in a dig at Romney: "I will say, I've spent a lot of time in Detroit lately, and I didn't want to let go Detroit go bankrupt. So in this particular World Series, I might be a little partial."

With Mini SGCC gone, I don?t have to worry about preparing substantial meals with enough leftovers to feed a horde of hungry teenagers.?? While this means a lot less work for me, I kind of miss it.?? I don?t know.? Cooking for just the two of us just isn?t as much fun.? I?m sure I?ll eventually get back in the groove.? Until then, I?ll be serving up more ?one pot wonders? like this Pasta Fazool for Cavemen.

If you?ve been hanging with me for a while, you may have seen my grandmother?s? Pasta e Fagioli recipe that I wrote about several years ago.?? Pasta e fagioli, aka ?pasta fazool?, which means ?pasta and beans?, is a traditional Italian dish made with cannellini beans, garlic, onions, broth, and some type of small pasta, such as ditalini.??? Like many Italian classics, pasta e fagioli started out as a peasant dish, using inexpensive ingredients and featuring beans as a protein instead of meat.? Some versions are like a soup, and others more of a stew. Some use tomatoes. Some don?t.? Either way, it?s still considered cheap eats.??

Truth be told, I?d never heard of pasta e fagioli until? after I was married and started collecting cookbooks.? Hard to believe coming from an Italian girl from the Bronx, huh ? especially considering that I?ve been eating it all of my life???? In my neighborhood, pasta e fagioli didn?t exist -? but ?pasta fazool? did.??? That?s what our grandmothers call it.? That?s what our mothers called it.? And, that?s what we kids called it.? The Italian language is made up of many different regional dialects.? Few speak the standard textbook version.? In fact, some of these dialects are so different, that sometimes native Italians from different parts of the country even have a hard time understanding each other!?? The word for beans in the Neapolitan and other southern Italian dialects is pronounced ?fazool?.?? The families in the area where I grew up were predominantly southern Italian.? Ergo, we ate pasta fazool.

Now, I?ll bet you?re wondering where the cavemen part fits into all of this.? Well, as I mentioned above, pasta fazool is traditionally a meatless dish.? That?s why it?s called pasta fazool.? Otherwise, it would be called? pasta e carne, which is pasta and meat.? But, some people here at Chez SGCC are cavemen,? (like the person I?m married to),? and don?t consider a meal to truly be a meal unless it contains meat.?? I kid you not.? No matter how substantial or filling a vegetarian offering I set before him may be, to him -? it?s still a side dish.? Sigh??? Sometimes, I just tell him ?tough beans?.? Sometimes, I humor him and throw him a meaty bone.??? And other times, like this one, I secretly thank him for his pig-headedness, because adding meat to this dish turned out to be a very, very good idea.

For my caveman-style pasta fazool, I used some nice, organic, hot Italian sausage.? Think of it as an Italian version of pork and beans.? I removed the sausage meat from the casings and browned it up in a deep skillet.

Then, I saut?ed some carrots, celery, onions and garlic in all of that tasty, rendered pork fat.?? Sound good so far?

I also tossed in some fresh thyme, rosemary and a bay leaf, and let them work their magic.

After that, I added? the fazool and smushed about a third of them around in the pan.

Pasta fazool wouldn?t be the same for me without some diced tomatoes.

Then, in goes the broth and some water.? I like low-sodium chicken broth, but you can use vegetable or beef broth too.? Bring it up to a boil and then stir in the pasta.

Reduce the heat and let everything get all cozy together until the pasta is tender. The starch released from the pasta thickens up the broth to the perfect ?stewy? consistency, and you?ve got the most delicious, homey, hearty, meaty, stick-to-your-ribs pasta fazool evah.?? It doesn?t get much easier ? or tastier ? than that, folks!

Pasta Fazool for Cavemen

Ingredients

  • 2 tablespoons olive oil
  • 1 pound Italian sausage meat, removed from the casings
  • 1 large sweet onion, chopped
  • 1 carrot, diced
  • 1 rib celery, diced
  • 3 large cloves garlic, minced
  • 4 sprigs fresh thyme
  • 1 large sprig fresh rosemary
  • 2 (14.5-ounce) cans cannellini beans, drained and rinsed
  • 1 cup diced plum tomatoes, canned or fresh
  • 4 cups chicken broth
  • 2 cups water
  • 1 bay leaf
  • 2 cups ditalini pasta or another kind of short pasta
  • Salt and freshly ground black pepper to taste
  • Pinch red pepper flakes, optional
  • 1/2 cup freshly grated Parmesan
  • 1-2 tablespoons extra-virgin olive oil for drizzling

Directions

  1. Heat olive oil in a large stock pot or Dutch oven over medium-high heat.? Crumble the sausage into the pot and cook until browned.? Remove with a slotted spoon and set aside in a bowl.
  2. Reduce heat to medium and add the onions, carrot and celery.? Saut? for about 5 minutes, until they begin to soften.? Add garlic, thyme and rosemary and saut? a few minutes more, until herbs become fragrant.
  3. Mash 2/3 cup of the cannellini beans and add to the pot, mixing well., and continue to saut? for about 2 minutes.? Then, add the rest of the beans and the tomatoes to the pot and saut? for 2-3 minutes more.
  4. Add the broth,water, bay leaf and reserved sausage meat? to the pot, stir and bring to a boil.? Add the pasta, reduce heat and simmer for about? 15 minutes, uncovered, until pasta is cooked and cooking liquid has thickened.
  5. Remove from heat and season with salt, pepper and red pepper flakes.? Add the freshly grated Parmesan and finish with a drizzle of extra-virgin olive oil. Serve warm with some crusty Italian bread.

Serves 6 as a main course.

UK Olympians appeal for help with stolen medals

LONDON (AP) ? Two British Olympians appealed Thursday for help in recovering a pair of bronze medals that were snatched from a London nightclub, while police reported an arrest in the case.

Rower Alex Partridge and field hockey player Hannah Macleod issued public appeals for the return of the medals, which they earned during the 2012 London Olympics. The thefts reportedly occurred at the Mahiki nightclub, where the athletes went following a reception at Buckingham Palace hosted by Queen Elizabeth II in honor of Britain's Olympic team.

"I'm absolutely gutted that my Olympic bronze medal has been stolen," Macleod said in a statement. "Winning it in August was an incredible highlight for me, and for this to have happened is simply shattering."

Macleod was a member of the women's team that defeated New Zealand, 3-1.

"While it means everything to me, it has no value to anyone else, and I just hope that whoever took it can hand it in or leave it somewhere that means I can get it back," she said.

Partridge, a member of the men's eight rowing team, said his team blazer was also taken.

The jacket was later handed in at a northwest London police station, but not the medal, according to police.

Police investigating the alleged theft of the medals said a 31-year-old man had been arrested and was later released on bail.

(AP) ? A deadly flare-up in fighting between Israel and Gaza's militant Hamas group subsided on Thursday after Egypt helped to restore calm ahead of a major Muslim holiday.

Weeks of simmering violence had intensified on Tuesday night, when rocket fire from Gaza drew Israeli airstrikes that killed two Palestinian militants. The hostilities came to a boil Wednesday, when militants fired some 80 rockets and mortars at southern Israel, and Israeli aircraft struck Gaza four times. In all, four Palestinians, including three militants, were killed in the fighting and two foreign workers in Israel were critically wounded.

The rocket and mortar fire stopped altogether overnight, though one projectile landed in southern Israel on Thursday morning, causing no damage. The military said it last struck Gaza on Wednesday morning. The violence ebbed as Muslims began preparing for the Eid al-Adha holiday, which begins on Friday.

Both sides confirmed Egyptian involvement in ending the fighting.

Under longtime leader Hosni Mubarak, Egypt had played an important role in halting multiple outbreaks of hostilities between Israel and Gaza militants. The new government of Egyptian President Mohammed Morsi who belongs to Hamas' parent movement, the Muslim Brotherhood, kept up the tradition.

Israeli defense official Amos Gilad told Army Radio on Thursday that Egyptian security forces have "a very impressive ability" to convey to the militants that it is in their "supreme interest not to attack."

Hamas spokesman Ayman Taha said Egypt conveyed Israel's desire to contain the violence.

"We said we'll abide by the calm if the occupation abides," he said. "It happened over the phone with Egyptian intelligence."

Hamas, which has killed hundreds of Israelis in suicide bombings and other attacks, has largely avoided attacks since a devastating Israeli military offensive nearly four years ago. It remains virulently anti-Israel but has sought to keep things quiet as it consolidates control of Gaza, which it violently overran five years ago.

Still, it is under pressure from smaller groups to prove that it remains in confrontation with the Jewish state.

Israeli Foreign Minister Avigdor Lieberman suggested the visit Tuesday of Qatari's leader to Gaza ? the first by a head of state to the territory since it came under Hamas rule ? emboldened Hamas to clash this week with Israel by bolstering the group's legitimacy.

No online game seem to be safe from the coming Halloween, with many MMORPGs getting special events and themed characters modifications. Today is TERA?s turn to get its fair share of pumpkins and ghosts, as shown by the trailer depicting some of the planned Halloween special events released today.

The Exiled Real Of Arborea, abbreviated as TERA, is a MMORPG developed by Bluehole Studio released in South Korea in 2011 and this past May in the west. A Japanese version is currently in the works, while other localized versions are currently being rumored.
The game has typical MMORPG characteristics such as crafting, player versus player actions and questing. The combat system, called Non Target battle system since the player targets foes with a cross-hair cursor, is a real time system with third person view where the players actively have to dodge enmey attacks.
Characters in the game belong to one of the seven races available, ranging from staples like Human and Elves to the Amans, draconian humanoids, the Castanics, demon-like and a few other.
Characters classes include typical fantasy games? professions like Priest, Sorcerer, Warrior, Berserker, Archer and some more, eight classes total.

The Trailer showcases some of the Halloween Events: one of the events stars the Pumpkin headed monster Gorudo that can be defeated only by throwing pie pieces at him; in the next event shown event players are required to defend baskets of candies from monsters? assaults!

Changing epidemiology of clostridium difficile

ScienceDaily (Oct. 22, 2012) ? A stay in the hospital may not be the only way to acquire Clostridium difficile diarrhea -- but the potentially life-threatening infection may be associated with a number of health complications in hospitalized children, according to the findings from two studies unveiled October 22 at the American College of Gastroenterology's (ACG) 77th Annual Scientific meeting in Las Vegas. In a separate case report also presented October 22, fecal microbiota transplantation in a 20-month old with recurrent Clostridium difficile infection (CDI) suggests the therapeutic potential fecal bacteriotherapy in pediatric patients who fail standard therapy for CDI.

In one study, researchers from Sinai Hospital in Baltimore retrospectively analyzed the changing epidemiological trends of patients with Clostridium difficile-associated diarrhea (CDAD) who were admitted to an acute care hospital between January 2005 and December 20120. The results of their analysis, "Changing Epidemiology of Clostridium difficile-Associated Diarrhea (CDAD) Among Long-Term Care Facility Patients," suggest a changing shift in the way CDAD is acquired -- from a traditional hospital-acquired infection to a community and long-term-care facility-based infection.

For the study, CDAD was defined as having clinical signs and symptoms of Clostridium difficile infection and a positive c. difficile stool toxin assay. The patients in the study were divided into three groups: nosocomial (hospital-acquired); long-term-care facility (LTCF); and community-acquired. Of the 258 toxin-positive CDAD patients in the analysis, 53 (20.6 percent) were nosocomial (hospital acquired); 119 were LTCF (46.1 percent) and 86 (33.3 percent) were in the community. The mean age for LTCF patients was higher than the other groups. Presenting symptoms were divided into diarrhea and non-diarrheal symptoms including fever, abdominal pain, and altered mental status. According to the results, the incidence of acute diarrhea was significantly lower in LTCF (18) patients as compared to patients from community (25).

Among the other findings:

* Most LTCF patients (80) presented with non-diarrheal symptoms whereas 47 community patients had non-diarrheal symptoms

* Use of proton pump inhibitors (PPI)s was more frequent in LTCF patients (73 percent) and nosocomial patients (69.8 percent) as compared to community patients (43 percent) -more-

* No clear indication was found for PPI use in 24.13 percent of LTCF patients using PPIs (21 out of 87), compared t 12.9 percent (4 out of 35) of nosocomial patients and 32.1 percent (9 out of 37) of community.

"There seems to be an epidemiological shift in C. difficile associated diarrhea from a traditional hospital -acquired infection to a community and long-term care facility-based infection," said co-investigator Vivek Kumar, M.D. He noted that diarrhea was not found to be the main presentation in LTCF patients. "This finding suggests that suspicion of C. difficile associated diarrhea should be high even if they present with non-specific symptoms such as abdominal pain, fever and altered mental status." He adds that more large scale epidemiological studies are needed to help us understand the spread of CDAD in the community.

Study Suggests CDI Associated with Increased Length of Hospital Stay, Other Health Implications for Children

While hospital and population-based studies have shown an increased incidence of C. difficile infection (CDI) in both adults and children, relatively little is known about the outcomes of infection in hospitalized children with CDI, according to researchers from the Mayo Clinic who analyzed the National Hospital Discharge Survey (NHDS) database to determine the epidemiology and outcomes of CDI in hospitalized children in another study. "The Epidemiology and Outcomes of Clostridium difficile Infection in Children from 2005-2009: Results from a Nationwide Survey," suggest that despite increased awareness of CDI in children and advancements in the management of CDI and infection and control practices, CDI remains a "major problem" in hospitalized children, and is associated with increased length of stay, colectomy, in-hospital mortality and discharge to a short-or-long-term care facility (DTCF).

There were an estimated 13.7 million children over the five-year study period, with a median age of 5 years and 47.8 percent female. Of this group, there were about 46,176 CDI cases (0.34 percent of all pediatric admissions) with a median age of 3 years; 48.1 percent female. The annual rate of CDI varied from 2.4 to 4.3 cases per thousand over the study period with no significant trend up or down, according to co-investigator Sahil Khanna, M.D.

Among the key findings children with C. difficile infection, compared to controls, had:

? Longer median length of stay (LOS): 6 versus 2 days

? Higher rates of colectomy (1.6 percent versus .32 percent)

? Higher all-cause in-hospital mortality (1.2 percent versus 0.48 percent) ? Higher discharge to long-term care facility (4.3 percent versus 2.7 percent)

"We did find that children with C. difficile infection were likely to stay in the hospital four days longer than children without the infection; twice as likely to lose part or all of their colon, and two and a half more times as likely to die in the hospital," said Dr. Khanna. "Despite increased awareness of C. difficile infection in children and advancements in management of C. difficile infection and infection prevention and control practices, this study suggests that CDI remains as major problem in hospitalized children. After adjusting for age, sex and cormorbidities, this study also suggests that C. difficile infection was an independent and the strongest predictor of increased length of stay, higher rates of colectomy, higher all-cause in-hospital mortality and higher discharge to a long-term care facility," Dr. Khanna explained.

"In hospitalized children C. difficile infection can be a major cause of morbidity and mortality. CDI should be considered as a differential in hospitalized children with diarrhea and managed aggressively," added Dr. Khanna.

Fecal microbiota therapy to treat recurrent C. difficile infection

"Recurrent C. difficile infection (CDI) has become a major problem in children often requiring recurrent and prolonged course of antibiotics," said co-investigator Dr. Sudhir Dutta. In a separate case-report also presented October 22, Dr. Dutta and a team of physicians from Sinai Hospital-Johns Hopkins Program in Internal Medicine and the Pediatric GI Department reported success after using fecal microbiota therapy to treat a case of recurrent C. difficile infection in a 20-month old male who was born premature at 27 weeks of gestation.

Fecal Microbiota Transplantation (FMT) involves taking the stool of a healthy person and putting it into the colon of a person with disease to help restore health. In persons with C. difficile infection, the goal is to restore the natural balance of good and bad bugs in the gut and eliminate the recurrent diarrhea, which can be life-threatening.

"While fecal microbiota therapy is successfully being used in adults with recurrent C. difficile infection, to our knowledge there is only one reported case in the pediatric literature where treatment with fecal microbiota transfer resulted in spontaneous improvement of symptoms and improvement of diarrhea," said Dr. Dutta.

In this case, the patient presented with a two-month history of diarrhea, rectal bleeding, and failure to thrive with a past medical history that included chronic lung disease and gastroesophageal reflux disease as well as treatment with multiple courses of antibiotics.

Co-investigator Dr. Rita Batra, of Sinai's Pediatric GI Department, noted that the decision to conduct FMT in the 20-month old patient was due to the severity of symptoms and because the procedure, "allows the fastest means of colonic composition of its normal microbiome components thus providing a primary line of defense against colonization and proliferation of opportunistic pathogenic bacteria including C. difficile."

After receiving donor stool from his mother via colonoscope in the right colon, the patient has now remained symptom-free with complete resolution of diarrhea, rectal bleeding and has consistently gained weight over the past three months, according to the case report.

"This case demonstrates the therapeutic potential of fecal bacteriotherapy in pediatric patients who fail standard therapy for C. difficile infection," said Dr. Dutta. "Randomized controlled studies with long-term follow up are needed to support the efficacy and safety of fecal microbiota therapy in pediatric patients."
